Report Concerns about ISU Animal Research

Concerns can be reported by:

• Submitting the Animal Care Adverse Event Form (preferred method), 
• Contacting the IACUC Chair,
• Contacting the Office for Research Integrity and Compliance,
• Contacting the Animal Care Facility,
• Directing the concern to the Animal Care Email,
• Anonymously reporting via the Maxient web site, or 
• Anonymously reporting to Maxient over the phone at: 1-800-461-9330

All concerns will be investigated and taken seriously.
