Medication Therapy Management

Medication Therapy Management (MTM) is a service that is provided for patients who have multiple disease states, take multiple medications, and who may not be compliant with their current therapy.  Every year, a patient who qualified will have a comprehensive medication review (CMR) by a pharmacist.  During the medication review, the pharmacist will go over the patient's medications, discuss what they are for, and address any concerns the patient may have.  After the review, the patient receives an updated medication list and an action plan of potential changes they could make or could be discussed with their doctor at their next visit.  The pharmacist can also contact the doctor on the patient's behalf to bring up any concerns they had before the patient sees their doctor.  MTM services at ISU offer a unique opportunity for both pharmacists and students.  Pharmacists can utilize their clinical skills, have more interaction with patients, and see a direct impact on a patient's life.  Students can build on their learning from the classroom about medications, interactions and refine their patient counseling skills, while working closely with a licensed pharmacist.  In addition, research opportunities are available to disseminate information about MTM's impact on patient care outcomes.
