Each year, all Faculty and Staff will participate in an evaluation process. The format of the process depends on your status and/or years of service.  There are 7 categories of evaluations and deadlines are different for each category.  Well in advance, you will receive an email from the Dean's office with your category, deadlines and instructions for completing and submitting your evaluation materials. If your email contains the wrong category for your evaluation, please contact the Dean's office immediately to resolve.

 

Faculty in Year 1 (Includes partial year) Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Select 1st Year as Evaluation Type.   Please include any IPE activities where applicable. 
  2. Provide electronic or hard copy version to your Department Chair by requested due date.

 

Faculty in Year 2 Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Select 2nd Year as Evaluation Type.   Please include any IPE activities where applicable.
  2. Provide electronic or hard copy version to your Department Chair by requested due date.         

 

Faculty in Year 3- TENURE TRACK Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Select 3rd Year as Evaluation Type.  Please include any IPE activities where applicable. 
  2. Provide electronic or hard copy version to your Peer Review Committee.
  3. Complete ISU P&T application and provide to yourxxxxxxxxxxxxxxxxxxxxxxxxxxxxx
  4. Provide a minimum of an (1) updated CV, a (2) summary table and (3) self-assessment
    narratives of instruction, scholarly activities, service and clinical practice (where appropriate) to your Peer Review Committee as requested.
  5. Faculty may request peer evaluation of teaching or may include any other information (as outlined in the ISU Periodic Performance Review Faculty Evaluation Form) for consideration.

Department Chair will complete

  • Addendum to Faculty Evaluations Rating Sheet
  • KDHS Third Year Review Form

The following MUST be submitted to KDHS:  Addendum to Faculty Evaluations Rating Sheet, KDHS Third Year Review Form, ISU P&T Application, CV, Job Description (02-18)

 

Faculty in Year 3- NON-TENURE TRACK Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Please include any IPE activities where applicable. 
  2. Provide electronic or hard copy version to your Department Chair by requested due date.

 

Faculty in Year 4 Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Please include any IPE activities where applicable.
  2. Provide electronic or hard copy version to your Department Chair by requested due date.

 

PPR  Faculty in Year 5 (or multiples of 5)

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Select PPR as Evaluation Type.  Please include any IPE activities where applicable.  Provide electronic or hard copy version to your Peer Review Committee by requested due date.
  2. Provide a minimum of an (1) updated CV, a (2) summary table and (3) self-assessment
    narratives of instruction, scholarly activities, service and clinical practice (where appropriate) to your Peer Review Committee as requested.
  3. Faculty may request peer evaluation of teaching or may include any other information (as outlined in the ISU Periodic Performance Review Faculty Evaluation Form) for consideration.

Department Chair will complete

  • Addendum to Faculty Evaluations Rating Sheet
  • Periodic Performance Review form

The following MUST be submitted to KDHS: Addendum to Faculty Evaluations Rating Sheet, Peer Review Cover Letter from Dean’s office, CV, Periodic Performance Review form (02-18)

 

Promotion & Tenure Candidates

  1. Submit a letterof intent with an updated CV, summary table of professional activities, summary of teaching evaluations and list of suggested reviewers to your Department Chair, Dean Cady and the Faculty Affairs Committee by requested due date. 
  2. Update your Activities in Activity Insight. Please include any IPE activities where applicable.

 

ALL OTHER FACULTY (not year 1-4, a PPR year (multiples of 5) or a P&T year) Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Select Regular Annual as Evaluation Type.   Please include any IPE activities where applicable.
  2. Provide electronic or hard copy version to your Department Chair by requested due date.

 

Department Chair Annual Faculty Evaluation

  1. Update your activities & complete your Annual Faculty Evaluation Part I in Activity Insight. Please include any IPE activities where applicable.
  2. Provide electronic or hard copy version to your DEAN by requested due date:

 NOTE:  Chair will complete Activity Insight Part 1, Dean will complete Activity Insight Part 2, VP will complete Activity Insight Part 3

 

Non-Classified/Classified Evaluations

Non Classified and Classified staff will receive an email from Human Resources with instructions and deadlines.  Complete your evaluation in Talent Management by the requested due date. 

 

 

Activity Insight

To update your Activities ONLY

  1. Please log onto Bengal Web,
  2. Click on ACADEMICS,
  3. Click ACTIVITY INSIGHT on the Institutional Research tab, and log in.
  4. Click each item in MANAGE ACTIVITIES to add recent information to applicable fields.  Click SAVE.

To complete your Annual Faculty Evaluation or PPR:

  1. Please log onto Bengal Web, 
  2. Click on ACADEMICS,
  3. Click ACTIVITY INSIGHT on the Institutional Research tab, and log in.
  4. Click each item in MANAGE ACTIVITIES to add recent information to applicable fields.  Click SAVE.
  5. Select RUN REPORT (the information you just added will prefill into applicable areas)
  6. SELECT DHS Annual Evaluation.
  7. SELECT Date range: 01/01/20XX to 12/31/20XX
  8. SELECT TO INCLUDE an Individual (select yourself)
  9. Select type of evaluation from the options (PPR, or specific year) & update any applicable fields in Part 1 with your recent information 
  10. Complete questions VIII, IX (1-6) and X (don't forget to check a box)
  11. Then, save it as a Word document, and provide a copy of Part 1 to your chair (hardcopy or electronic)
