COVID-19 Healthcare Provider Impact Survey
The COVID-19 pandemic has had an affect on all residents of Idaho. At Idaho State University, the Graduate Nursing Faculty are interested in how the pandemic has had an impact on you as a healthcare provider. You are invited to participate in a research study by completing an electronic survey that will take approximately 7-10 minutes of your time.
Your responses to this survey are anonymous. However, you may provide a preferred email address to receive follow up surveys. This is the only time your email address will be requested and implies your willingness to participate in the follow-up surveys. The email address you provide will be stored on a HIPPAA compliant server, be password protected, encrypted and only accessible by two study investigators. It will be used only for the purposes of sending two follow-up surveys three and six months after you complete this survey. Email addresses will be erased from the server upon conclusion of data collection. The emails with the link to the follow up surveys will be sent using the same HIPPAA compliant platform. The email address you provide will be deleted entirely from the server upon completion of data collection.
Your completion of the survey implies your consent. You may stop at any time and do not have to answer any question within the survey. You may also choose not to participate in the study. Data will be reported in aggregate form such that no one provider can be identified based upon their responses to the survey(s).
