Researchers working with animals are required to complete a health & safety Risk Assessment as part of the IACUC (Institutional Animal Care and Use Committee) protocol approval process.
Environmental Health, Safety, and Sustainability (EHSS) works collaboratively with researchers to identify protocol hazards and effective control measures. Both the hazards and control measures necessary to protect health and safety are documented on the Risk Assessment form.
Researchers initiate the IACUC Risk Assessment review and approval process by completing the form below. EHSS should be contacted if assistance is needed to complete the form. After the researcher completes the form and presses "submit", the form is sent electronically to EHSS for review. EHSS will either approve the Risk Assessment as written, contact the researcher for additional information, or request that the researcher make changes to the Risk Assessment form. EHSS forwards approved Risk Assessments to the IACUC.
