Common questions about the application process

Filling out the application

Question: What if I am currently enrolled in a preference class(es) this spring semester? Can I take credit for them on the application?

Answer: No, points are not awarded to courses that have not been completed by the Application submission deadline. However, please identify which preference classes you are currently enrolled in on your application.


Question: How do I submit my application?

Answer: You can hand deliver your application to Students Services in the College of Technology (Building #48), or mail it in to the following address:

Question: Do I need to submit all of the components of the application (Application, Letters of Recommendation, Observation Hours Documentation forms) together, at the same time?

Answer: It is recommended that you submit all the components together to ensure articles are not lost in the mailing process. You can submit the various components separately, but make sure to follow-up with student services to ensure that correspondence has been received.


Letters of Recommendation:

Question: Who can complete my Application Reference Forms?

Answer: The PTA Program requires (2) Application References: The Application References can be completed by any of the following persons: PT, PTA, Healthcare Provider, Professor, Instructor, Advisor or Employer. There should be no familial relation between the student and person providing the reference.


Question: Is there a specific format that should be used for the attached Letters of Recommendation to the Application Reference Form?

Answer: Yes, the person you have chosen to complete the Application Reference can also attach a letter of recommendation to the form. The attached letter should be submitted in 12 point font, single spaced and be a maximum length of 2 pages. Both the Application Reference form and attached written letter of recommendation must be placed in an envelope with your name on it. The person who completed the Application Reference Form and Letter of Recommendation should seal the envelope containing the two documents and sign over the closure.

Answer: The Application Reference forms are located on the PTA homepage under the PTA Application tab. You can hand deliver your Application Reference forms to Students Services in the College of Technology (Building #48), or mail it to the following address:

Observation hours:

Question: How do I submit proof of my Observation hours?

Answer: Forms used to document Observation hours are located on the PTA homepage under the PTA Application tab. You can hand deliver your application to Students Services in the College of Technology (Building #48), or mail it to the following address:

Question: What if I applied to the PTA program last year (2014) and have 40 hours of observation from just 1 facility? Do I need to do 20 more hours in a different facility?

Answer: Yes, this year (2015) we have made the transition to our new Observation hours policy. You may use 20 observation hours completed previously, however you will need to do 20 hours in an additional facility, for a total of 40 hours.
